We are located at 10250 SE 167th Place RD, in Summerfield, FL, 34491. This is across the street from Baylee Plaza on 441, north of WalMart. The practice is located in Lakeview Internal Medicine, Suite 5-2 (the next to last door). You will recognize it by the big electronic sign out front.
